Rocky Mountain Associated Physicians Weight Loss Surgery Requirements

The requirement for all patients to receive surgery through Rocky Mountain Associated Physicians includes the following:

• Attendance at "The Benefits & Risks of Weight Loss Surgery” free information seminar. Patients who live outside a reasonable travel distance to Salt Lake City, will receive the information in the mail.
• Complete the patient information form
• Complete the medical history form
• Consultation with the Surgeon
• Obtain all insurance required medical record documentation
• Obtain history and physical with a letter from a primary care doctor giving clearance for surgery
• Copies of insurance card and photo ID
• RMAP will prepare and submit your application, medical records and a letter of medical necessity to your insurance company for written approval.
• Authorization takes 6-12 weeks depending on your insurance
• Attendance at a support group is strongly encouraged prior to surgery.
