Rocky Mountain Associated Physicians Weight Loss Surgery Requirements

The requirement for all patients to receive surgery through Rocky Mountain Associated Physicians includes the following:

  • Attendance at "The Benefits & Risks of Weight Loss Surgery" free information seminar. Patients who live outside a reasonable travel distance to Salt Lake City, will receive the information in the mail.
  • Complete the patient information form.
  • Complete the medical history form.
  • Consultation with the Surgeon.
  • Obtain all insurance required medical record documentation.
  • Obtain history and physical with a letter from a primary care doctor giving clearance for surgery.
  • Copies of insurance card and photo ID.
  • RMAP will prepare and submit your application, medical records and a letter of medical necessity to your insurance company for written approval.
  • Authorization takes 6-12 weeks depending on your insurance.
  • Attendance at a support group is strongly encouraged prior to surgery.

Free Informational Class

Rocky Mountain Associated Physicians offers a free instructional seminar called The Benefits and Risks of Weight Loss Surgery several times each week. The seminar gives details about Weight Loss Surgery. The risks, benefits, and qualifications for the surgery are reviewed.

Patients are instructed in how to complete the required steps to obtain the surgery through Rocky Mountain Associated Physicians.
