Rocky Mountain Associated Physicians Weight Loss Surgery Requirements
The requirement for all patients to receive surgery through Rocky Mountain Associated Physicians includes the following:
- Attendance at "The Benefits & Risks of Weight Loss Surgery" free information seminar. Patients who live outside a reasonable travel distance to Salt Lake City, will receive the information in the mail.
- Complete the patient information form.
- Complete the medical history form.
- Consultation with the Surgeon.
- Obtain all insurance required medical record documentation.
- Obtain history and physical with a letter from a primary care doctor giving clearance for surgery.
- Copies of insurance card and photo ID.
- RMAP will prepare and submit your application, medical records and a letter of medical necessity to your insurance company for written approval.
- Authorization takes 6-12 weeks depending on your insurance.
- Attendance at a support group is strongly encouraged prior to surgery.
